Electronic Tutoring Referral Form

In order to ensure that there are enough weeks remaining in the semester, please submit your nine-week referral by the end of the third week of the term.

To receive an electronic report from the consultant after each session, student consent must be procured. A student can verbally consent at the tutoring session, or you can print out the paper (PDF) form of this document and have the student sign the form and bring it with them to the Writing Center.
